I confirm that I have received an intact and functioning HP Probook 450 laptop computer including software and the following accessories:  laptop bag, carry strap, charging cable, earphones, mouse. 

1.  I agree to use the computer only on the library premises and return it to the circulation desk no later than three hours after receiving the computer.
2. If I do not return the computer in the allotted time, I will be fined 5 shekels an hour.  In the event that the computer is not returned at all (including if lost), I will be charged $1000 for the value of the computer (at the current rate of exchange in NIS).
3. a. I promise to return the computer and its included accessories in the same working condition as I received them.
   b. Additionally, I promise to return the computer without any additional files or software installed. 
4.  The university will not be liable for any damage or loss to the lender do to the use of the computer.
5. The computer is my sole responsibility and I will not give the computer to anyone else or give permission for anyone else to use the computer.
   1. The student’s behavior, in the event of disciplinary proceedings (if the computer is stolen and/or damaged not due to the negligence of the borrower) can serve as a consideration to the disciplinary committee who may impose penalties at its discretion.
   2. I am aware that a breach of my obligation is a violation of the University’s policy.
6.  I know that the University is not responsible for keeping my documents or information saved on the computer and that it is my sole responsibility to save these documents or information.
7. I am aware that files and/or software that are installed on the computer will be deleted upon shutting down the computer.
